Joining
the Practice
New patients wishing to register with the practice must live in our catchment area (see HERE) and complete the application pack (available at reception or download - Links at bottom of page). We will require your NHS medical card or your passport or birth certificate with one of the following: 1) a current utility bill, 2) tenancy card or lease agreement or 3) a bank or building society statement. The name on the documents must be the registrant's name unless you can provide a marriage cetificate or deed of change of name. All those over 16 years of age will need to book in for a health check. Overseas Visitors. Non-UK residents are advised to check whether they are entitled to NHS treatment. Foreign nationals from EU states can be seen on the NHS. Those from non-EU countries do not have an automatic right to NHS care. Those in the UK for less than 6 months will have to be seen as private patients. Your nationality or payment of UK taxes and NI contributions are not relevant. Temporary Residents. We are happy to see UK residents who are visiting or temporarily residing with our patients. However we will only be able to deal with their acute problem. |
